Fluid and Electrolytes in the Aged

In this article, the last in a four-part series, we explore fluid maintenance with particular reference to disorders of the pituitary gland. Fluid management techniques and related nursing care are also covered. Body fluids are mainly water and electrolytes, and the three main organs that regulate fluid balance are the brain, the adrenal glands and the kidneys Tortora and Grabowski, One-third of the total is circulatory fluid, sometimes known as extracellular fluid ECF ; the remainder is intracellular fluid ICF contained within cells Docherty and McIntyre, ; Edwards The anatomy and physiology of homeostasis are covered in part one of this series. Most tissues contain a lot of water bones and adipose tissue are the two main exceptions. It has many functions, including Tortora and Grabowski, ; Adam and Osborne, :.

Summary: Clinical case scenarios in a problem-based format! It is well organized, concise and appropriate to the target audience. Clincal case studies are an invaluable resource in the education and continuing education of students of medical and clinical science and Clinical Cases in Fluid and Electrolyte Balance: An Acute Care Approach is no exception and is a quality addition to the currently available texts. Key features of each case study include: -clinical and physiological learning objectives -a case timeline running throughout each case conveys a realistic view of patient management -clinical and physiological commentaries running in parallel to the timeline provide a vital link between the clinical and biomedical science concepts -review questions for self-assessment and an ideal examination aid -topics for further discussion are included for motivated students. The prescription and administration of intravenous fluid therapy, and the ordering of pathology tests are some of the most frequent acts performed by hospital staff. Intravenous therapy, however, can be complicated and it is essential that practitioners have an understanding of the appropriate use and interpretation of the most basic pathology tests.

The regulation of body fluid balance is a key concern in health and disease and comprises three concepts. The first concept pertains to the relationship between total body water TBW and total effective solute and is expressed in terms of the tonicity of the body fluids. Disturbances in tonicity are the main factor responsible for changes in cell volume, which can critically affect brain cell function and survival. Solutes distributed almost exclusively in the extracellular compartment mainly sodium salts and in the intracellular compartment mainly potassium salts contribute to tonicity, while solutes distributed in TBW have no effect on tonicity. The second body fluid balance concept relates to the regulation and measurement of abnormalities of sodium salt balance and extracellular volume. Estimation of extracellular volume is more complex and error prone than measurement of TBW. A key function of extracellular volume, which is defined as the effective arterial blood volume EABV , is to ensure adequate perfusion of cells and organs.

Fluid and Electrolyte Balance in Children

Care of the Critically Ill Patient pp Cite as. The major function of the cardiorenal system is to maintain the fluid bathing the cells at a constant composition, for it is through this pericellular fluid that O 2 and nutrients are delivered and waste products of metabolism removed. The early marine vertebrates had an extracellular fluid similar to that of their salt water environment, but with evolution to fresh water and land there were various modifications, which included the development of an integument impermeable to water and modifications in renal function to allow the excretion or conservation of water and sodium according to the prevailing conditions. This chapter will outline the distribution of water and electrolytes throughout the body and the aspects of circulatory and renal physiology which are responsible for maintaining this steady state. The Regulation of Fluid and Electrolyte Balance

Electrolyte imbalance , or water-electrolyte imbalance , is an abnormality in the concentration of electrolytes in the body. Electrolytes play a vital role in maintaining homeostasis in the body. They help to regulate heart and neurological function, fluid balance , oxygen delivery , acid—base balance and much more. Electrolyte imbalances can develop by consuming too little or too much electrolyte as well as excreting too little or too much electrolyte. Electrolyte disturbances are involved in many disease processes, and are an important part of patient management in medicine.

The relationship of age, serum creatinine level, and creatinine clearance. Curves are standardized for a kg man with variable baseline creatinine values by means of the Cockroft-Gault formula 11 adapted from Beck 3. Fluid balance concepts in medicine: Principles and practice
